Right to change legal gender: Legal, but requires surgery from past to now.
In January 2019, the European Court of Human Rights (ECHR) ruled that North Macedonia's requirement that transgender people undergo sex reassignment surgery before their gender marker on ID documents can be changed is a violation of human rights. The judgment held that deficiencies in North Macedonia's laws concerning modifying gender on official identification documents infringed the private life of transgender Macedonians.

In July 2016, the Bosnian Parliament adopted a bill amending anti-discrimination laws to explicitly prohibit discrimination based on sexual orientation, gender identity and sex characteristics.

On 19 October 2018, the Federation of Bosnia and Herzegovina (FBiH) adopted a request for the legalisation of civil unions. The Government will now form a working group to analyse the proposal, which will need to be later adopted by the Parliament.

On 26 March 2009, Parliament approved a unified anti-discrimination law, known as the Anti-Discrimination Act of 2009, which prohibits, among other categories, discrimination on the basis of sexual orientation and transgender status in all areas.
