DescriptionSince May 2026, Tennessee has begun making a yearly public list of trans people that access official routes of gender-affirming care as mandated by HB 754 signed by Governor Bill Lee on May 7th and effective the same day. The law requires; the name of the prescriber, the dosages, frequency of dosages, any trans affirming surgeries, and mental health history to be reported to the health department for that database, any clinic that does not comply with giving that data could be fined 150k USD and their prescriber to lose their license for half a year, requires the state health department to make annual reports of said data, prohibits counties from banning conversion therapy, and requires prescibers and medical insurance to provide "detransition care" while making medical transition harder.

Ambetter Insurance, owned by the largest provider of Medicare and Medicaid insurances in the nation, has stopped covering hormones and other transition medicine for trans adults. This is the first known insurance company to deny care in a large swath of states as the broader trend of restricting gender-affirming care continues.

Transgender individuals across the country have been reporting more denials by their insurance companies for gender-affirming care; federal employees, military veterans including their families have reported that the federal government is no longer covering gender-affirming care and new rules which go into effect beginning 2027 will make the restriction permanent unless blocked by a court.

Gender-affirming care for minors continue to see more restrictions even in states historically known for vast protections under state law. In Illinois, A university denied an 18 year old trans woman's hormones citing Trump's executive order from 2025 "banning" gender-affirming care for minors under 19. In New York, NYU Langone continues to shutter care for trans youth despite outcry from AG James and a federal court opinion which barred Trump from withholding federal funds to the clinics. However, a subpoena would be issued by the Northern District of Texas mandating NYU Langone and other clinics across the country to release all private medical information regarding the trans youth patients including their families. A court case is currently pending out of New York intending to halt the subpoena as of June.

The Colorado Supreme Court would mandate in mid-May that trans youth clinics must continue their care within the state and sue the Trump Administration if such care is threatened to be shuttered. A district court in Kansas would re-legalize gender-affirming care for minors after a 2025 ban was ruled unconstitutional in the injunction.
